Today, I want to discuss tuberous breast surgery—what it entails, why it may be necessary, and the associated costs.

Understanding the Diagnosis

One of the first questions that arises upon recognising a diagnosis of tuberous breasts is, “How can I get this treated?” This is often followed by queries about available options, the number of surgeries required, the nature of these procedures, the necessity of breast implants, potential scarring, and, importantly, the overall cost.

Unfortunately, providing a straightforward answer is challenging, as each case is unique. Decision-making is influenced by your specific circumstances, including personal preferences regarding breast implants and the extent of scarring you’re willing to accept.

The Trade-Offs Involved

Like all plastic surgeries, correcting tuberous breasts involves certain trade-offs. These include balancing the degree of shape correction against the invasiveness of the operation and the resultant scarring. Most corrections will involve scars, at least around the areola—the pigmented area surrounding the nipple. In some cases, additional vertical or transverse scars in the breast crease may be necessary.

Techniques for Correcting Tuberous Breasts

Each breast may have developed differently, necessitating tailored approaches for correction. While I aim to address tuberous breast deformities in a single operation, I always advise patients to anticipate the possibility of a second procedure. This could be less invasive, such as fat grafting to enhance breast contour or add volume to the lower pole, or more extensive, depending on the initial outcome.

Below are approximate costs for typical tuberous breast correction procedures in my practice:

  • Circumareolar mastopexy (unfurling flaps): £9,475
  • Circumareolar augmentation mastopexy with anatomical implants: £11,237
  • Augmentation mastopexy with anatomical implants: £12,597
  • Fat grafting (lipofilling): £6,305–£7,290

It’s important to note that fat grafting, also known as lipofilling, is a common adjunctive procedure for secondary adjustments in tuberous breast correction. This can often be performed as a day case with a relatively quick recovery.

What Causes Tuberous Breasts?

During puberty, some young women notice that their breasts don’t develop like those of their peers, leading to feelings of confusion and concern. Questions like “Why me?” and “What causes this?” are common.

The exact cause of tuberous breast deformity remains unclear. I’ve encountered cases where only one family member is affected, as well as instances where sisters share similar breast development patterns.

One consistent observation is the disparity in skin quality: the breast skin tends to be thick and less elastic, while the areolar skin is thinner and less supportive. This imbalance can lead to herniation of breast tissue into the areola, resulting in the characteristic tuberous shape. A tight, well-defined breast crease further restricts normal development of the lower breast pole.

Correcting Tuberous Breasts

In my experience, tuberous breast correction involves addressing two main areas: the areola and the inframammary fold (breast crease).

If the breast crease is in a favourable position—not too high or tight—maintaining its location often yields the most predictable results, avoiding potential lower pole shape issues. However, if the fold is significantly high and tight, surgical intervention to lower it may be necessary. This can be achieved without implants, using internal reconstruction techniques like unfurling flaps or fat grafting. Alternatively, implants combined with radial scoring (internal incisions to release tight bands) can facilitate expansion and reshaping.

Areolar correction typically involves reducing its size and addressing puffiness or herniation. This is usually accomplished with internal stitching techniques, often employing permanent sutures to maintain the new shape and prevent recurrence.

Personalising the Surgical Approach

Combining areolar and lower pole corrections varies between individuals. Some surgeons may adopt a one-size-fits-all approach upon diagnosing tuberous breasts. However, I strongly believe in analysing each case individually to determine the most appropriate surgical plan tailored to the patient’s unique anatomy and goals.

Our primary objective is to achieve a natural breast shape, enhance symmetry, and enable patients to wear clothing confidently. Ultimately, the goal is to restore body confidence and normalcy.

Quality of Life Post-Surgery

I aim to alleviate the stress and anxiety associated with tuberous breasts, empowering patients to regain confidence in various aspects of life—be it social activities, clothing choices, or intimate relationships. The improvement in self-esteem following surgery is often remarkable, making it one of the most rewarding procedures I perform.

While future life events like pregnancy, weight fluctuations, or ageing may necessitate additional interventions, the positive impact on a patient’s mindset and quality of life is profound and enduring.
